How to Get Vaxcert San Juan

Step-by-Step How to Get a VACCINE CERTIFICATE Online

1.      Go to the Vaccination Record Portal.

       Click the link: https://vaxcert.doh.gov.ph

2.      After clicking the link it will appear on the home page. 

3. Click the continue button to request for a Vaccination Certificate.

4. Review and accept the data privacy statement.

Check the checkbox “I have read and agreed to the Privacy Statement”. Then Click the     continue button

5. Enter your Personal Information. Make sure they match the details on your vaccination card.

• Last Name
• First Name
• Middle Name
• Birthdate (MM/DD/YYYY)
• Vaccination Details

1st Dose Vaccination 

• Municipality/City of Vaccination & Vaccination Date

2nd Dose Vaccination & Vaccination Date

• Municipality/City of Vaccination & Vaccination Date

Additional Dose Vaccination

• Municipality/City of Vaccination & Vaccination Date

2nd Additional Dose Vaccination

• Municipality/City of Vaccination & Vaccination Date

Check that I’m not a robot then click the next button.

6. Review the Information you entered and click the continue button.

7. If your information is correct. click Generate Vaccination Certificate.
